Success Development Specialist (Case Manager)

The Success Development Specialist is responsible for assisting community members with obtaining sustainable employment. S/he is responsible for guiding trainees through the training process by identifying and seeking resolutions for potential and existing barriers including navigating community health services, psychiatric services, educational services, treatment programs, and housing placement for trainees and graduates to access. Monitors graduates adherence to program guidelines and progress 1 year after job placement that include retention checks and assistance with support services (access to training, job replacement, transportation, or any other support services as needed).

Summary of Essential Job Functions (include, but are not limited to)

Case Management + Career Readiness Assistance:

  • Complete comprehensive barrier assessment and goal strategy activities with trainees.
  • Work alongside Career Development Specialist to create a career development plan for each trainee.
  • Mentor trainees through any remaining aspects of their career development plans.
  • Ensure trainees remain employed for longer than 90 days.
  • Track and provide support to graduates for one year after employment placement.
  • Refer participant to strategic partners, as needed.
  • Maintain soft (SharePoint) and hard copy (Physical File) file system as per organizational requirements.
  • Complete data entry, track and maintain trainee profiles in the Apricot database, and other specified database systems
  • Complete reports, case notes, and other written documents to maintain compliance and ensure program impact
  • Meet with each trainee a minimum of once a week providing one-on-one case management/mentor session.

Training Development:

  • Develop and maintain relationships with outside agencies who may provide workshops, training, and supportive resources to trainees and graduates.
  • Develop and maintain relationships with outside agencies who may be a referral source to the program.
  • Assist trainees to develop career plans, determine training and employment needs, and acquire financial assistance for training.
  • Assist trainees to become work-ready by identifying barriers and offering resources/solutions.
  • Conducts orientation, interviews, screens, and counsel trainees for Power52 training programs.
  • Participate in trainee enrollment process for training programs.
  • Prepare and maintain Power Principle lesson plans. Lesson plans should be prepared in advance of the cohorts.
  • Deliver daily Power Workshops to cohort trainees (30 minutes).

Impact Measurement:

  • Maintain participant drop logs (probation/expulsion); submit to Management bi-weekly.
  • Maintain database for 1:1 assessment for Management review on a monthly basis.
  • Ensure appropriate documentation is complete and maintained for each participant.
  • Contribute to the program as needed.

Other (As needed):

  • Contribute to program reporting, as needed.
  • Contribute to grant application, meetings, and reporting, as needed.
  • Attend all meetings required by management.

Qualifications:

  • Bachelor’s Degree preferred
  • Minimum two years’ experience with case management for adults considered high-risk
  • Experience working with returning citizens, substance abusers, and those who may have gang affiliation
  • An understanding of adversities faced by adults with legal infractions. Assist in dealing with trainee probation officers.
  • Minimum two years’ experience with program development
  • Proficient knowledge of workforce programs
  • Excellent verbal and written communication skills required
  • Ability to foster a cooperative work environment
  • Ability to establish and maintain effective working relationships with staff members and stakeholders
  • Exceptional with time management, able to multi-task, and meet deadlines
  • Able to work efficiently with minimal supervision
  • Strong communication skills, both written and verbal
